Job Summary
We are seeking a highly skilled and experienced Clinical Discharge Lead to join our team at University Hospitals Dorset NHS Foundation Trust. As a key member of our Discharge Service, you will play a vital role in ensuring the safe and timely discharge of patients from our acute hospital sites.
The successful candidate will have a proven track record of leadership and influencing skills, with the ability to enthuse, motivate and involve individuals and teams. You will be responsible for coordinating referrals to our Transfer of Care Hub (TOCH) and working closely with internal and external partners to achieve effective discharge planning.
This is an exciting opportunity for a dynamic and autonomous practitioner to develop their skills in complex discharge management and coordination, and to enhance their negotiation skills. You will have the opportunity to work in a multidisciplinary team, consisting of nurses, therapists, and administrative staff, and to contribute to the continued development of our Dorset Discharge model.
