Clinical Discharge Lead

We are seeking an experienced and skilled Clinical Discharge Lead to join our team at University Hospitals Dorset NHS Foundation Trust. The successful candidate will be responsible for leading the discharge planning process, ensuring that patients receive timely and effective care, and that hospital resources are utilized efficiently.
Key Responsibilities:- Develop and implement discharge planning strategies to improve patient outcomes and reduce hospital length of stay
- Lead a multidisciplinary team to coordinate discharge planning, including nurses, therapists, and administrative staff
- Collaborate with internal and external partners to ensure seamless transitions of care
- Monitor and evaluate the effectiveness of discharge planning processes and make recommendations for improvement
- Provide education and training to staff on discharge planning best practices
- Registered Nurse or AHP with relevant experience in an acute hospital setting
- Proven leadership and influencing skills, with the ability to motivate and involve individuals and teams
- Effective communication and interpersonal skills, with the ability to communicate complex information to a wide range of staff
- Ability to work independently and as part of a team, with a strong focus on patient-centered care
- Excellent time management and organizational skills, with the ability to prioritize tasks and manage multiple projects
