About the Role

We are seeking a skilled and experienced Clinical Assessment and Treatment Service Nurse to join our team at Dorset HealthCare. As a key member of our clinical team, you will be responsible for providing high-quality patient care and contributing to the development of our services.

Key Responsibilities

  • Assess and treat patients with complex health needs, working collaboratively with other healthcare professionals to ensure comprehensive care.
  • Develop and implement individualized care plans, taking into account the unique needs and circumstances of each patient.
  • Provide education and support to patients and their families, promoting health and well-being and empowering them to take an active role in their care.
  • Contribute to the development and implementation of clinical policies and procedures, ensuring that our services are safe, effective, and responsive to patient needs.
  • Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of solutions.

Requirements

  • Registered practitioner to degree/diploma level, supplemented by post-registration diploma level specialist training and/or short courses or demonstrable extensive experience in the relevant specialty.
  • Experience at Practitioner Band 5 level, with a strong understanding of clinical governance and quality improvement principles.
  • Ability to communicate effectively at different levels of the organisation, with staff, patients, and external organisations, both verbally and in writing.
  • Experience of planning and organising complex activities, including discharge planning and coordination with other agencies.
