About the Opportunity

We are looking for a dedicated Care Coordinator to join our Care Outlook team in London. As a key member of our team, you will play a crucial role in delivering high-quality care services to our customers.

Responsibilities and Accountabilities
  1. Develop and implement effective care plans, ensuring seamless coordination between the care team and service users.
  2. Directly supervise and mentor care staff, focusing on their development and performance improvement.
  3. Monitor and evaluate care assessments, risk planning, and customer feedback to maintain high standards of care.
  4. Respond to emergency situations, providing immediate care services when required.
  5. Contribute to quality assurance initiatives, working closely with the Branch Manager to address concerns and improve services.
  6. Collaborate with colleagues to achieve departmental goals, promoting a positive and supportive work environment.
Requirements and Qualifications
  • Degree-level qualification in Health and Social Care or a related field (e.g., NVQ Level 3).
  • Proven experience in care coordinating or a related role within Health and Social Care.
  • Knowledge of CQC regulations and standards.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • A full driving license and access to a vehicle (preferred).
