Richmond and Wandsworth Councils are seeking a highly skilled and experienced Health and Social Care Team Lead to join our team. As a dynamic and inspirational leader, you will play a vital role in delivering high-quality health and social care services to our residents.

The successful candidate will have a professional qualification in Social Work or Occupational Therapy and registration with SWE/HCPC. They will also possess demonstrable post-qualification experience of working in a statutory Social Care or Health setting with adults, as well as knowledge of current developments in relevant legislation, national guidance, and frameworks.

We offer a competitive salary of £56,000 per annum, which reflects the demands and responsibilities of this role. In addition, we provide a range of benefits, including 40 days annual leave (including Bank Holidays), flexible working, and a generous pension plan.

You will be responsible for overseeing the effective management of hospital discharges from acute and non-acute settings, building positive working relationships with our Health and community partners, and responding to a change in circumstances due to a hospital admission.

As a Health and Social Care Team Lead, you will deliver effective risk management, using your knowledge of legislation and guidance in relation to hospital discharge and continuing health care, to promote well-being. You will also be expected to communicate clearly, verbally and in writing, including the ability to present issues so that adults, carers, other professionals, and members of the public are able to understand them.
