Job Overview


We are seeking an experienced and skilled Physiotherapist to join our team as an Advanced Physiotherapist in the Integrated Community Stroke Team.


As a key member of our multidisciplinary team, you will provide high-quality, evidence-based rehabilitation services to stroke survivors in both acute and community settings.


Key responsibilities include:


Main Duties


Assess and treat stroke survivors in acute and community settings


Support discharge planning and family/carer communication


Attend daily/weekly multidisciplinary team meetings


Provide goal-led rehabilitation and delegate work as required


Oversee complex patients and support junior staff as needed


Work collaboratively with the wider multidisciplinary team to deliver high-quality care

Person Specification


Essential:



  • BSc degree or postgraduate diploma in Physiotherapy
  • Registered with the HCPC
  • Evidence of relevant postgraduate training or experience
  • Previous relevant experience at band 6 or 7
  • Current knowledge and experience of patients with conditions relevant to the specialty

Desirable:



  • Membership of an appropriate specialist interest group
  • Change management experience and ability to develop expert practice
