Adult Support Planner

We are seeking a skilled and compassionate Adult Support Planner to join our specialist Learning Disabilities & Autism Service. This role involves providing a social work service to an allocated caseload, working closely with individuals, carers, and other professionals to deliver effective support and interventions.
Main Responsibilities:- To assess individual needs and develop tailored support plans, promoting independence and well-being.
- To coordinate services with other professionals, ensuring seamless delivery and effective outcomes.
- To maintain accurate records, adhering to regulatory requirements and organisational policies.
We offer a competitive salary of £35,000 - £42,766 per annum, depending on experience, along with a range of benefits, including contributory pension scheme, annual leave entitlement, and training opportunities.
