This role is part of a specialist service for adults with complex needs, focusing on strength-based assessments, person-centered support planning, and long-term support arrangements.
As a social worker, you will be required to carry out Mental Capacity Assessments and Court of Protection work, ensuring the highest standards of service are achieved.
Key Responsibilities

- Act as a member of a team, delivering strength-based assessments, person-centered support planning, and brokerage services to adult service users, carers, and their families.
