About the Role

This Adult Support Planner position is an exciting opportunity to make a positive impact on the lives of individuals with complex needs. As part of our specialist Learning Disabilities & Autism Service, you will be responsible for delivering a high-quality social work service to an allocated caseload, ensuring that each individual receives tailored support to meet their unique requirements.

Responsibilities:
  1. To conduct comprehensive strength based assessments, identifying areas of strength and resilience in individuals, and developing tailored support plans to promote their independence and well-being.
  2. To work closely with other professionals, including healthcare and education teams, to ensure seamless coordination of services and effective outcomes.
  3. To maintain accurate and up-to-date records, adhering to regulatory requirements and organisational policies.
