About the Position

The role involves working in a dynamic team environment, providing supervision and support to junior staff, and ensuring a seamless flow for patient care between hospital and community settings.

Key Responsibilities
  • To provide high-standard patient care in hospital settings and patients' homes.
  • To ensure a seamless flow for patient care between hospital and community settings, enabling patients to be cared for in their own home environment.
  • To provide supervision and support to more junior staff.
  • To triage referrals to the community team.
