We are seeking a skilled and compassionate individual to join our team as a Supported Pathway Officer. In this role, you will work closely with a Local Authority in Southwark to provide high-quality customer-focused support to residents.

Key Responsibilities:

  • Provide intensive assertive keyworking support using reflective practice techniques to assist residents in reaching set goals and progressing along an identified clear pathway for housing.
  • Coordinate necessary support services to assess residents' ability to live independently and identify services that can assist them in sustaining their wellbeing and tenancy.
  • Encourage residents to engage and participate in activities that address issues of social isolation and confidence building, and accompany them where necessary and appropriate.
  • Advocate for residents where required, providing practical support if needed to ensure their rooms are habitable and safe, and accompany them to appointments as necessary.

Requirements:

  • Knowledge of the impact of Adverse Child Experiences and the principles of Trauma-Informed Care when dealing with homeless people who have complex needs.
  • Knowledge of the Care Act and Housing legislation and the available support in relation to single homeless adults.
  • Knowledge of welfare benefits and other support available to residents of supported accommodation.
  • Health and Safety issues in relation to a supported housing setting.

Experience:

  • Experience of working with homeless people with complex support needs.
  • Experience of recovery-focused and harm minimisation service provision for clients with multiple and complex needs.
  • Experience of risk assessment, keyworking, and support planning.
  • Experience of case coordination in a multi-agency setting.

  • This role requires an enhanced DBS.
