Pathway Support Specialist

We are seeking a skilled and compassionate individual to join our team as a Supported Pathway Officer. In this role, you will work closely with a Local Authority in Southwark to provide high-quality customer-focused support to residents.
Key Responsibilities:
- Provide intensive assertive keyworking support using reflective practice techniques to assist residents in reaching set goals and progressing along an identified clear pathway for housing.
- Coordinate necessary support services to assess residents' ability to live independently and identify services that can assist them in sustaining their wellbeing and tenancy.
- Encourage residents to engage and participate in activities that address issues of social isolation and confidence building, and accompany them where necessary and appropriate.
- Advocate for residents where required, providing practical support if needed to ensure their rooms are habitable and safe, and accompany them to appointments as necessary.
Requirements:
- Knowledge of the impact of Adverse Child Experiences and the principles of Trauma-Informed Care when dealing with homeless people who have complex needs.
- Knowledge of the Care Act and Housing legislation and the available support in relation to single homeless adults.
- Knowledge of welfare benefits and other support available to residents of supported accommodation.
- Health and Safety issues in relation to a supported housing setting.
Experience:
- Experience of working with homeless people with complex support needs.
- Experience of recovery-focused and harm minimisation service provision for clients with multiple and complex needs.
- Experience of risk assessment, keyworking, and support planning.
- Experience of case coordination in a multi-agency setting.

- You should be available to work immediately or at short notice.
- You should have the right to work in the UK.
- This role requires an enhanced DBS.
