Pay Rate: £24 Per Hour

The Role:

We are seeking a skilled Pathway Support Specialist to join our team at Novax Recruitment Ltd. As a Pathway Support Specialist, you will play a vital role in assisting residents in securing their next stage of housing and providing resettlement support to ensure a successful transition into their new home.

Key Responsibilities:

  • Provide assertive client-centered key-working to a caseload of approximately 10 clients, focusing on supporting them to maintain their current accommodation and link into necessary support services.
  • Conduct risk assessments and develop support plans to address the unique needs of each client.
  • Offer floating support for 6 months, ensuring clients are linked into services and meaningful activities that promote their physical and mental well-being, as well as life skills.

Work Pattern:

The successful candidate will work a shift pattern that includes early and late shifts, Monday to Friday, with shifts on a rota basis. This will provide a dynamic and challenging work environment that requires flexibility and adaptability.
