About the Role

The successful candidate will be responsible for coordinating care services for individuals with complex needs, ensuring that their physical, emotional, and social needs are met. This involves developing and maintaining relationships with service users, families, and other stakeholders.

Key Responsibilities:

  • Conducting assessments and developing personalized care plans
  • Coordinating and scheduling care services, including medication management and personal care
  • Ensuring compliance with regulatory requirements and quality standards

Requirements:

  • Experience working in social care, preferably in a coordinating or supervisory role
  • Strong communication and organizational skills
  • Able to work flexibly, including evenings and weekends
  • Hold a relevant qualification, such as SVQ or equivalent
