Community Health Services Lead

This is an exciting opportunity to join our Community Health Services team as a Nursing Team Leadership Position. As a leader, you will be responsible for managing a team of Community Nurses who provide palliative care to patients in the community.
Key Responsibilities- Provide leadership and guidance to the team, ensuring delivery of high-quality care to patients and their families.
- Support teams in ensuring the provision of palliative care services in the community, working collaboratively with other healthcare professionals and agencies.
- Develop and implement strategies to improve patient outcomes and quality of care.
- Mentor and supervise junior staff members.
- Registered Nurse with extensive experience in palliative care or related specialties.
- Strong leadership and management skills, with a proven track record of leading successful teams.
- Excellent communication and interpersonal skills.
- NMC (Adult Nurse) Registered.
Salary Range: £26,500 - £33,100 per annum (full-time equivalent).
