About the Role

We are seeking an experienced and highly motivated Clinical Nurse Specialist to join our Specialist Palliative Care Team at Northampton General Hospital.

The successful candidate will be responsible for managing a caseload of patients with specialist needs, providing advanced practice in planning, implementing, and evaluating nursing care, and working collaboratively with the wider multi-disciplinary team.

Main Responsibilities

  • Act as a core member of the Specialist Palliative Care multidisciplinary team, facilitating the effective management of patients referred to the team.
  • Undertake a comprehensive holistic patient assessment, ensuring that results of patients' holistic needs assessment are considered when making decisions.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient need and in accordance with evidence-based practice and national and local protocols.
  • Provide Level 2 psychological support for patients and their carers, referring patients with complex needs to the Macmillan Clinical Psychologist if necessary.
  • Act as a specialist resource to other healthcare professionals relating to Specialist Palliative Care, providing expert nursing advice and acting as a role model demonstrating high standards of holistic care.

Requirements

  • In-depth specialist palliative and end of life care experience, demonstrating experience of caring for palliative patients in an acute setting.
  • Clear up-to-date knowledge of current statutory frameworks and national policy.
  • Extensive post-registration experience, with the ability to manage a caseload and work as part of a multidisciplinary team.
  • Ability to provide education to all levels of staff, with evidence of experience of holistically assessing specialist palliative and dying patients in an acute hospital setting.
