About the Role
We are seeking a skilled and compassionate Holistic Nursing Specialist to join our Community Specialist Palliative Care Team. As a key member of our team, you will provide holistic nursing care to patients requiring symptom management and end-of-life care in their own homes.
Main Responsibilities
- Assess patient needs and symptoms, implementing individualized care plans and providing education to patients and their families.
- Collaborate with the multidisciplinary team to ensure a coordinated approach to patient care.
- Develop and maintain effective relationships with patients, families, and healthcare professionals.
Requirements
- Registered Nurse (RGN) with experience in palliative care and community nursing.
- Excellent communication and interpersonal skills, with the ability to work autonomously and as part of a team.
- Knowledge of symptom management and end-of-life care, with a passion for delivering high-quality patient care.
