About the Role

We are seeking a skilled and compassionate Palliative Care Nurse to join our Community Specialist Palliative Care Team in Northamptonshire. As a key member of our team, you will provide holistic nursing care to patients and their families, supporting them through the end-of-life journey.

Key Responsibilities
  • Assess patient needs and symptoms, implementing individualized care plans and providing education to patients and their families.
  • Collaborate with the multidisciplinary team to ensure a coordinated approach to patient care, utilizing effective communication and interpersonal skills.
  • Develop and maintain relationships with patients, families, and healthcare professionals, promoting a culture of empathy and compassion.
  • Participate in audits and quality improvement initiatives, contributing to the development of best practices in palliative care.
Requirements
  • Palliative Care Qualification and relevant experience.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Knowledge of clinical governance and quality issues, with a commitment to evidence-based practice.
  • Understanding of holistic nursing care and symptom management for patients approaching the end of life.
