About the Role
We are seeking an experienced and highly motivated Clinical Nurse Specialist to join our Specialist Palliative Care Team at Northampton General Hospital. The successful candidate will have a strong background in palliative care and be passionate about delivering high-quality patient care.
Key Responsibilities
- Act as a core member of the Specialist Palliative Care multidisciplinary team, facilitating the effective management of patients referred to the team.
- Undertake a comprehensive holistic patient assessment, ensuring that results of patients' holistic needs assessment are considered when making decisions.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient need and in accordance with evidence-based practice and national and local protocols.
- Provide Level 2 psychological support for patients and their carers, referring patients with complex needs to the Macmillan Clinical Psychologist if necessary.
- Act as a specialist resource to other healthcare professionals relating to Specialist Palliative Care, providing expert nursing advice and acting as a role model demonstrating high standards of holistic care.
Person Specification
- Essential:
- In-depth specialist palliative and end-of-life care experience, demonstrating experience of caring for palliative patients in an acute setting.
- Clear up-to-date knowledge of current statutory frameworks and national policy.
- Extensive post-registration experience.
- Ability to manage a caseload and work as part of a multidisciplinary team.
- To have provided education to all levels of staff.
- Desirable:
- Evidence of research, publications, conference presentations.
- Evidence of local audit, participation in or leadership of.
