Palliative Care Specialist

We are seeking an experienced and highly motivated Palliative Care Specialist to join our dynamic team at Northampton General Hospital. As a key member of our Specialist Palliative Care Team, you will play a vital role in delivering high-quality care to patients and their families.
Key Responsibilities- Manage a caseload of patients with specialist needs, working collaboratively with the wider multi-disciplinary team.
- Attend the weekly Specialist Palliative Care multidisciplinary team meeting, contributing to the discussion and patient assessment/care planning.
- Undertake a comprehensive holistic patient assessment, ensuring that results of patients' holistic needs assessment are considered when making decisions about patient care.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient need and in accordance with evidence-based practice and national and local protocols.
- Provide Level 2 psychological support for patients and their carers, referring patients with complex needs to the Macmillan Clinical Psychologist as necessary.
- Knowledge and Experience
- Essential:
- In-depth specialist palliative and end of life care experience.
- Clear up-to-date knowledge of current statutory frameworks/national policy.
- Extensive post-registration experience.
- Ability to manage caseload and work as part of an MDT.
- To have provided education to all levels of staff.
- Evidence of experience of holistically assessing specialist palliative and dying patients in an acute hospital setting.
