Main Responsibilities

The successful candidate will play a vital role in providing holistic care to patients and their families, working closely with the Community Specialist Palliative Care Team. Key responsibilities include:

  • Assessing patient needs and symptoms, and implementing individualized care plans.
  • Providing sensitive and compassionate communication with patients and their families, ensuring their emotional and psychological needs are met.
  • Working collaboratively with the team to ensure a coordinated approach to patient care.

Requirements

To be successful in this role, you will need:

  • Palliative care qualification and experience of working in a community setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work flexibly, including evenings, nights, and weekends, and to travel across Northamptonshire as required.
