About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our CAMHS Crisis Home Treatment Team. As a key member of our team, you will play a vital role in providing high-quality care and support to children and young people with mental health difficulties.

Key Responsibilities
  • Provide home treatment interventions for children, young people, and their families, promoting positive mental health and well-being.
  • Work collaboratively with the multi-disciplinary team to ensure effective clinical decision-making and coordination of care.
  • Develop and maintain strong relationships with service users, families, and carers, empowering them to take an active role in their care and treatment.
  • Contribute to the development and implementation of individualized care plans, ensuring that the needs of each service user are met.
  • Participate in regular case discussions and reflective practice sessions to ensure high-quality care and continuous improvement.
  • Adhere to Trust policies and procedures, ensuring the safety and well-being of service users and staff.
Requirements
  • Professional qualification in Nursing, Social Work, or Occupational Therapy.
  • Excellent communication and interpersonal skills, with the ability to work effectively with children, young people, and their families.
  • Strong understanding of child and adolescent mental health, with knowledge of relevant policies and procedures.
  • Ability to work flexibly and adapt to changing circumstances, with a strong commitment to delivering high-quality care.
  • Valid driving license and access to a car, with the ability to travel to various locations as required.
