About the Role

We are seeking a highly motivated and compassionate Community Health and Wellbeing Worker to join our team at Hedena Health Ltd. As a key member of our team, you will play a vital role in improving the health and wellbeing of our community.

Key Responsibilities
  • Conduct regular household visits to assess the health and social needs of individuals and families.
  • Identify vulnerable households and individuals and provide support and advocacy to help them access appropriate services.
  • Work collaboratively with healthcare teams and other agencies to provide patient-centred, integrated care.
  • Develop supportive relationships with local community groups and statutory services to make timely and appropriate referrals.
  • Provide health coaching and social prescribing to promote healthy lifestyles and wellbeing.
Requirements
  • Good standard of education.
  • Experience of working and communicating with the public.
  • Ability to communicate effectively and build trust with members of the public.
  • Knowledge of the local community and its demographics.
  • Strong interpersonal and communication skills.
