About the Role

We are seeking a highly motivated and compassionate Community Health and Wellbeing Worker to join our team at Hedena Health Ltd. As a key member of our team, you will play a vital role in promoting the health and wellbeing of our community.

Key Responsibilities
  • Conduct regular household visits to assess the health and social needs of individuals and families, adopting a proactive and holistic approach to support the local community.
  • Identify vulnerable households or individuals and work collaboratively with healthcare teams to provide targeted support and interventions.
  • Act as an advocate to help households navigate the healthcare system, access appropriate services, and remove barriers to accessing resources.
  • Develop supportive relationships with local community groups, statutory services, and voluntary sector providers to ensure seamless referrals and support.
  • Work collaboratively with colleagues across health, social care, and the third sector to provide patient-centred, integrated care.
Requirements
  • Good standard of education.
  • Experience of working and communicating with the public.
  • Ability to communicate effectively and build trust with members of the public.
  • Knowledge of the local community, its demographics, culture, local services, agencies, and resources.
  • Strong interpersonal and communication skills, social perceptiveness, and observational skills.
  • Ability to work effectively with a wide range of teams in a diverse community.
  • Commitment to advocate for social changes that promote the health and wellbeing of the local community.
