We are seeking a highly skilled and motivated Health and Wellbeing Coach to join our team at Maximus. As a Health and Wellbeing Coach, you will play a critical role in empowering individuals to take control of their health and wellbeing by providing personalized support and guidance.
Key Responsibilities- Deliver Health Coaching Services: Provide face-to-face, telephonic, and digital health coaching support to individuals from a wide range of community and healthcare settings.
- Behaviour Change Support: Support individuals in establishing and attaining health-promoting goals, focusing on lifestyle behaviour change, including healthy eating and nutrition, smoking cessation, physical activity, weight management, and alcohol consumption.
- Case Management: Manage caseloads efficiently, effectively, and at scale, ensuring accurate data collection and reporting in line with contractual and case management system requirements.
- Stakeholder Engagement: Engage with internal and external stakeholders, including local authorities, integrated care partnerships, community and voluntary sector organizations, and population being served.
- Continuous Professional Development: Commit to ongoing learning and professional development to ensure delivery of the most recent and up-to-date health coaching provision to the correct quality standard.
- Qualifications and Experience: Relevant health coaching qualification or an accredited health coaching skills programme, minimum of six months of direct health coaching delivery, and experience in delivering behaviour change interventions for specific lifestyle-related issues.
- Individual Competencies: Strong understanding of the social determinants of health, behaviour change principles and methodology, excellent oral and written communication skills, and ability to build and maintain relationships.
- Technical Skills: Proficient in the use of evolving digital technologies to support people through behaviour change processes, excellent Microsoft Office skills, and ability to learn quickly when using new systems and processes.
