Job Summary

We are seeking a highly motivated and compassionate Community Health and Wellbeing Worker to join our team at Hedena Health Ltd. As a Community Health and Wellbeing Worker, you will play a vital role in promoting the health and wellbeing of our community by providing support, guidance, and advocacy to individuals and families.

About the Role

The successful candidate will be responsible for:

  • Providing health and wellbeing support to individuals and families in the community
  • Advocating for the needs of our community members and connecting them with local resources and services
  • Working closely with healthcare professionals, social services, and community organizations to ensure seamless support and care
  • Developing and maintaining relationships with community members, organizations, and stakeholders to promote health and wellbeing initiatives
  • Collecting and analyzing data to inform health and wellbeing strategies and initiatives
Key Responsibilities

The successful candidate will be responsible for:

  • Conducting home visits and assessments to identify health and wellbeing needs
  • Developing and implementing personalized health and wellbeing plans
  • Providing education and support on healthy living, disease prevention, and management
  • Connecting individuals and families with local resources and services
  • Collaborating with healthcare professionals and social services to ensure seamless support and care
Requirements

To be successful in this role, you will need:

  • A good standard of education
  • Experience of working with vulnerable populations
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic IT skills
