Job Summary

We are seeking a highly motivated and compassionate Community Health and Wellbeing Worker to join our team at Hedena Health Ltd. As a key member of our team, you will play a vital role in improving the health and wellbeing of our community. Your primary responsibility will be to engage with members of the public, identify their health needs, and provide them with access to appropriate support and resources.

Main Duties and Responsibilities

Conduct regular household visits to assess the health and social needs of individuals and families.
Identify vulnerable households and individuals and provide them with targeted support and resources.
Act as an advocate for the community, helping them navigate the health and social care systems.
Develop and maintain relationships with local community groups, statutory services, and voluntary sector providers to ensure seamless referrals and support.
Collaborate with healthcare teams to identify health and social care needs and develop effective solutions.

Key Skills and Qualifications

Excellent communication and interpersonal skills.
Ability to work effectively with diverse communities and individuals.
Strong problem-solving and analytical skills.
Ability to work independently and as part of a team.
Basic IT skills and ability to use digital records.
