Job Summary

We are seeking a highly skilled and motivated Community Public Health Nurse to join our team at Oxford Health NHS Foundation Trust. As a key member of our 0-19 service, you will play a vital role in delivering high-quality care to children and families in our community.

About the Role

This is an exciting opportunity to work with a talented team of professionals who are passionate about improving the health and wellbeing of our community. As a Community Public Health Nurse, you will be responsible for delivering health promotion activities, conducting transitional assessments, and working in partnership with other agencies to support the health and wellbeing of children and families.

Key Responsibilities
  • Deliver health promotion activities to children and families in schools and community settings
  • Conduct transitional assessments and develop plans to support the health and wellbeing of children and families
  • Work in partnership with other agencies to support the health and wellbeing of children and families
  • Develop and maintain interagency networks to support the health and wellbeing of children and families
Requirements

To be successful in this role, you will need to have:

  • A First Level Registered Nurse or Midwife qualification
  • Practice assessor/supervisor experience
  • Excellent communication and interpersonal skills
  • Ability to work in partnership with other agencies
  • Knowledge and skills in IT packages, including Microsoft Word and Excel
