Job Title: Community Mental Health Nurse

We are seeking a highly skilled and experienced Community Mental Health Nurse to join our team at Oxford Health NHS Foundation Trust. As a Community Mental Health Nurse, you will play a vital role in providing high-quality care to older adults with mental health conditions.

About the Role

This is an exciting opportunity to work in a dynamic and supportive team, providing assessment and treatment interventions to adults over 65 years old. You will work closely with multidisciplinary teams, including doctors, nurses, occupational therapists, and support workers, to deliver person-centered care.

Key Responsibilities
  • Conduct comprehensive assessments of older adults with mental health conditions, identifying their needs and developing individualized care plans.
  • Deliver evidence-based interventions, including medication management, psychotherapy, and social support, to promote recovery and relapse prevention.
  • Work collaboratively with service users, their families, and carers to ensure their safety, well-being, and independence.
  • Participate in regular clinical team meetings and contribute to the development of new care approaches.
  • Supervise and support other staff members, promoting a culture of excellence and continuous learning.
Requirements

To be successful in this role, you will need:

  • Professional registration as a Mental Health Nurse with the Nursing and Midwifery Council (NMC).
  • Experience working in community mental health settings, particularly with older adults.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Knowledge of mental health legislation, policies, and guidelines, including the Mental Health Act 1983 and the Care Act 2014.
  • Ability to work autonomously and as part of a team, with a commitment to continuous learning and professional development.
