Community Nurse Opportunity

We are seeking a skilled Community Nurse to join our team at Health Professionals Nursing Agency. As a Community Nurse, you will provide high-quality nursing care to patients in their homes, promoting wellness and managing various health conditions.

Key Responsibilities:
  • Develop and implement individualised care plans tailored to patients' needs.
  • Monitor and assess patients' health status, ensuring timely intervention and adjustments to care plans as necessary.
  • Educate patients and families about self-care techniques, treatment plans, and available resources.
Requirements:
  • Registered Nurse (RN) with an active NMC license in the UK.
  • Demonstrated passion for community-based care and a strong commitment to patient well-being.
  • Excellent communication and interpersonal skills.
