Job Title: Paediatric Community Nurse

We are seeking an experienced Paediatric Community Nurse to join our team at Thornbury Community Services. As a Paediatric Community Nurse, you will be responsible for delivering high-quality care and support to young people with complex needs in a community-based setting.

Key Responsibilities:
  • Provide individualized care and support to young people with complex needs
  • Work collaboratively with multidisciplinary teams to develop and implement care plans
  • Conduct assessments and evaluations to identify the needs of young people and their families
  • Develop and implement individualized care plans to meet the needs of young people
  • Provide education and support to young people and their families on health and wellbeing
Requirements:
  • Minimum of 6 months experience in the last 2 years as a Paediatric Community Nurse
  • Recent experience in suctioning, mechanical cough assist, care of gastrostomy, and change of gastrostomy
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
