Job Title: Paediatric Community Nurse

Job Summary:

We are seeking an experienced Paediatric Community Nurse to join our team at Thornbury Community Services. As a Paediatric Community Nurse, you will be responsible for delivering high-quality care and support to young people with complex needs in a community-based setting.

Key Responsibilities:

  • Provide individualized care and support to young people with complex needs
  • Work collaboratively with multidisciplinary teams to develop and implement care plans
  • Conduct regular assessments and reviews to ensure the best possible outcomes for young people
  • Develop and maintain strong relationships with young people, families, and carers
  • Stay up-to-date with best practices and evidence-based guidelines in paediatric care

Requirements:

  • Minimum of 6 months' experience in paediatric care
  • Recent experience in suctioning, mechanical cough assist, and care of gastrostomy
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work varied shifts, including evenings and weekends
