Job Title: Paediatric Community Nurse

Job Summary:

We are seeking an experienced Paediatric Community Nurse to join our team at Thornbury Community Services. As a Paediatric Community Nurse, you will be responsible for delivering high-quality care and support to young people with complex needs in a community-based setting.

Key Responsibilities:

  • Provide bespoke care and support to young people with complex needs in a community-based setting.
  • Work collaboratively with multidisciplinary teams to develop and implement individualized care plans.
  • Conduct regular assessments and reviews to ensure the highest standards of care are maintained.
  • Develop and maintain strong relationships with young people, their families, and carers to ensure a seamless and coordinated approach to care.
  • Participate in ongoing professional development to stay up-to-date with best practices and evidence-based care.

Requirements:

  • Minimum of 6 months' experience in the last 2 years in a Paediatric Community Nursing role.
  • Recent experience in suctioning, mechanical cough assist, care of gastrostomy, and change of gastrostomy (emergency only).
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work a variety of shifts, including days, nights, and weekends.
