Community Health and Wellbeing Worker

We are seeking a highly motivated and compassionate Community Health and Wellbeing Worker to join our team at Hedena Health Ltd. As a key member of our team, you will play a vital role in promoting the health and wellbeing of our community.
Main ResponsibilitiesKey duties and responsibilities include:
- Conducting regular household visits to assess the health and social needs of individuals and families.
- Identifying vulnerable households and individuals and providing support and advocacy to help them access appropriate services and resources.
- Working collaboratively with healthcare teams and other agencies to provide patient-centred, integrated care.
- Developing supportive relationships with local community groups and statutory services to make timely and appropriate referrals for individuals and families.
- Providing health coaching and social prescribing to promote healthy lifestyles and wellbeing.

RequirementsTo be successful in this role, you will need:
