About the Role

Hedena Health Ltd is seeking a skilled Community Health and Wellbeing Specialist to join their team. As a key player in the community, you will be responsible for providing outreach support to individuals and families in need.

Main Responsibilities

  1. Conduct regular household visits to assess health and social needs, adopting a proactive and holistic approach.
  2. Identify vulnerable households and individuals, working closely with healthcare teams to provide support.
  3. Act as an advocate, helping households navigate the health and social care systems.
  4. Develop supportive relationships with local VCSE organisations, community groups, and statutory services.
  5. Liaise with healthcare professionals to promote health coaching and social prescribing.

Requirements

  • Good standard of education.
  • Experience of working and communicating with the public.
  • Ability to build trust and establish relationships with diverse individuals and communities.
  • Strong interpersonal and communication skills.
