About the Role

We are seeking a highly skilled and compassionate Mental Health Practitioner to join our team at Southern Health NHS Foundation Trust. As a key member of our Primary Care Network, you will play a vital role in delivering high-quality mental health services to our patients and communities.

Main Responsibilities
  • Work collaboratively with Primary Care and Secondary Mental Health Services to provide specialist advice and support, assessment, treatment, education, and solution-focused approaches to local Primary Care Teams.
  • Be professionally accountable and responsible for patient care, ensuring the highest standards of clinical practice and patient safety.
  • Triage and undertake initial assessments, signposting to appropriate services, or providing time-limited psycho-social therapeutic interventions directly in GP surgeries, patient's homes, and other primary care settings.
  • Maintain clinical knowledge and skills by attending training and applying this in practice to support the development of the team.
