Job Description

As a Practice Nurse at CroslandMoor Surgery, you will be responsible for delivering high-quality patient care and contributing to the overall success of our practice. Your duties will include:

  • Providing routine nursing care to patients in accordance with clinical-based evidence, NICE, and the NSF.
  • Identifying and managing patients at risk of developing long-term conditions, preventing adverse effects on their health.
  • Delivering chronic disease clinics and referring patients to secondary/specialist care as needed.
  • Maintaining accurate clinical records in conjunction with extant legislation.
