">We are a leading local authority, Cheshire West and Chester Council, seeking an experienced Social Services Coordinator to join our Hospital Discharge team. As a key member of our team, you will work closely with healthcare professionals, therapists, and nurses to ensure seamless transitions for patients from hospital to community care.
Salary and Package
">The salary for this role is £34,000 - £39,000 per annum, depending on experience. This position offers a competitive remuneration package that reflects the importance of this role in supporting vulnerable adults. Responsibilities
">As a Social Services Coordinator, your primary responsibilities will include:

- Conducting comprehensive assessments to identify individual needs and preferences;">
- Developing personalized care plans that prioritize patient choice and control;">
- Collaborating with multidisciplinary teams to ensure holistic approaches to patient care;">
- Maintaining accurate records and reports to inform decision-making;">
- Providing guidance and support to colleagues less experienced in social care processes;">
- Staying up-to-date with relevant legislation, policy, and frameworks governing adult social care.">
Requirements
">To be successful in this role, you will possess:
">- ">
- A CQSW/CSS/Diploma/Degree in Social Work or equivalent qualification;">
- Registration with Social Work England;">
- Able to undergo an enhanced DBS check and Adults Barred List check;">
- Excellent communication and interpersonal skills;">
- Ability to work effectively in a fast-paced environment;">
- Familiarity with relevant legislation, policies, and frameworks governing adult social care.">
Location and Culture
">This role is based within our Council offices in locations across the borough, with the option to work from home where suitable. You will have access to a supportive team culture and collaborative working environment.
