Main Responsibilities- Provide clinical care to young adults with diabetes, including assessment, diagnosis, and management of their condition.
- Work collaboratively with the multidisciplinary team to develop and implement individualized care plans.
- Support young adults with diabetes in managing their condition, including education on insulin therapy, blood glucose monitoring, and healthy lifestyle choices.
- Audit outcomes of care and initiate changes as necessary to improve patient outcomes.
- Develop and deliver structured diabetes patient education programs.
- Collaborate with healthcare professionals to develop innovative tools for the management of young adults with diabetes.
- Registered General Nurse with significant post-registration experience.
- Experience in multidisciplinary working and diabetes care.
- Ability to demonstrate an understanding of diabetes and insulin therapy.
- Excellent verbal and written communication skills.
- Ability to work independently and as part of a team.
