Job Title: Associate Paediatrics Diabetes Nurse Specialist

About the Role:

This is an exciting opportunity for a dedicated and experienced nurse to work with patients with complex diabetes, promoting their health and wellbeing through informed self-management.

Main Responsibilities:
  • Enhance Health and Wellbeing: Provide expert care to individuals with complex diabetes, helping them achieve optimal glycaemic control.
  • Empower Self-Management: Support and empower individuals to manage their diabetes effectively, promoting independence and confidence.
  • Educational Support: Offer education and guidance to individuals with diabetes and their carers, ensuring they have the knowledge and skills to manage their condition.
  • Foster Collaborative Working: Encourage and facilitate regular collaboration among all healthcare professionals involved in diabetes care, ensuring cohesive and coordinated care.
  • Facilitate Communication: Serve as a liaison between GP and consultant-led specialist teams, ensuring seamless communication and care.
Requirements:
  • Must have a degree/equivalent within Nursing (RGN qualified) and an active NMC Registration (Adult).
  • Specialist Diabetes qualification (Diploma).
  • Experience of working with patients with complex diabetes.
  • Recent UK nursing experience.
