About the Role

We are seeking a Community Care Worker to join our team on Sandford Ward, a male Older Adult Mental Health ward.

In this role, you will work collaboratively with clients to meet their care needs, promoting independence and supporting them in activities of daily living.

You will contribute to the assessment, planning, implementing, and evaluation of care and treatment, working closely with multidisciplinary teams to ensure seamless support.

Sandford Ward provides care for individuals with acute or functional mental illnesses, often with co-morbidities. Our team is dedicated to delivering high-quality care, prioritizing person-centered care and proactive treatment.

You will have the opportunity to develop your skills in crisis management, lead group and recreational activities, and work collaboratively with healthcare professionals to provide holistic care.
