Community Care Coordinator

We are seeking a skilled and experienced Paediatric Nurse Manager to join our team at Leaders In Care Recruitment Ltd. As a key member of our multidisciplinary team, you will be responsible for designing and delivering tailored care plans, managing multidisciplinary teams, and working closely with community teams and commissioners to establish bespoke packages of care.
About the JobThis is an exciting opportunity for a motivated and experienced Paediatric Nurse Manager to join our team and contribute to our mission of delivering high-quality care to children and their families.
Key Responsibilities- Design and deliver a tailored care plan specific to an individual client's needs.
- Effectively manage the multidisciplinary team through strong communication and teamwork.
- Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs within your caseload.
- Deliver nursing interventions, including the management of medicine delivery.
- Buid and maintain sophisticated relationships with clients' families.
- Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care.
- RSCN with a minimum of 6 months in the UK and an active NMC PIN.
- A supportive management style that can bring out the best in others.
- Up-to-date experience of tracheostomy care and ventilators within the last 2 years.
- Advanced communication skills verbally and written to liaise effectively with patients, families, and the MDT.
- Suitable for Band 5/Band 6 Nurses with evidence of effective leadership and management of HCAs.
- Full UK driving license desirable but not essential.
- Estimated salary: £49,000 per annum (dependent on experience).
- £3,000 car allowance.
- Two days working from home per week/ three days in the community.
- Monday to Friday (9-5) hours.
- 33 days holiday.
