Community Care Coordinator

We are looking for a dedicated Community Care Coordinator to join our team at Recovery Focus. As a valued member of our team, you will work with individuals to identify their needs and develop a personalised support plan that addresses these requirements. This may include assisting with employment, finances, accommodation, and social activities.
The successful candidate will have a caring and compassionate nature, empathy, and enthusiasm for helping others. A driving licence and access to a car are essential due to the nature of the role. No relevant experience is required, as full training will be provided, although NVQ level 3 Health & Social Care is an advantage. We welcome applications from those with lived experience and/or who are from a Black, Asian and Minority Ethnic background.
In return for your hard work and dedication, you will receive a competitive salary of £23,400 per annum, pro rata, as well as excellent training and development opportunities. You will also have the satisfaction of knowing that you are making a real difference in people's lives.
