Key Responsibilities:

  • Coordinate adult care services, ensuring timely delivery of high-quality care.
  • Support adults with complex needs, including those requiring crisis management and case coordination.
  • Maintain accurate records of adult care services, including assessments, interventions, and outcomes.
  • Collaborate with other professionals, including healthcare staff, carers, and family members, to achieve positive outcomes for adults with complex needs.

Requirements:

  • Hold a degree or equivalent in social work.
  • Be registered with Social Work England.
  • Have significant frontline experience in Adults Services.

Compensation and Benefits:
