As a highly skilled Care Coordinator with Acuity Care Group Limited, you will have the opportunity to make a real difference in the lives of vulnerable individuals.

We are looking for an experienced and dedicated individual to join our team as a Complex Care Manager. The successful candidate will be responsible for managing support packages with varying requirements, providing exceptional, person-centred care services, and ensuring that clients receive the highest standards of care and achieve their fullest potential.

The ideal candidate will have a genuine passion for delivering high-quality care, excellent communication skills, and a proven track record in managing and coordinating care for clients in a community setting. You will also need to be able to build and maintain strong relationships with clients, families, and other community stakeholders.

In this role, you will work closely with the Operations Manager and other internal departments to ensure seamless delivery of care services. You will be responsible for conducting thorough assessments, developing and implementing personalized care plans, and regularly reviewing and adjusting care plans to ensure they remain relevant and effective.

You will also be required to attend regular meetings with clients, their families, and integrated care boards (ICB) contacts, communicate effectively to ensure all parties are informed and involved in care decisions, and address any concerns or feedback to continuously improve the quality of care provided.

This is a field-based role involving regular travel across the Oxfordshire area and up to a 2-hour radius, as well as working from home. You will also be required to travel to the offices in Nottingham and Southampton for regular team meetings.

We offer a competitive salary of £37,000 per annum, plus a car allowance of £3,000 per annum, as well as 25 days annual leave + statutory bank holiday, pension contributions, and a range of benefits including 24/7 free access to a GP, health cash back plans, wellbeing programmes, and retail and supermarket discounts.

To be successful in this role, you will need to have a full UK driving license and access to your own vehicle, as well as proficiency in MS Office and the ability to learn new software systems quickly. Ideal candidates will also hold an NVQ Level 3 (or equivalent) in Health & Social Care or a related topic or be willing to study towards this.

Experience with Care Quality Commission (CQC) regulations and standards is essential, and familiarity with brain injury care would be advantageous but not required.
