Care Coordinator

Active Care Group's Care in the Home division continues to thrive, providing a safe, reliable, and outcome-based care to clients through our exceptional support workers, clinicians, and management team.
We are expanding our team of Care Coordinators and opening a new hub in the Oxford area, and we are looking for a skilled individual to join our friendly team.
As a Care Coordinator, you will be responsible for providing premium customer service, coordinating and scheduling care packages to ensure continuity of care for our clients in their own homes.
Our care in the home services support individuals with a wide range of conditions, including brain injury, spinal cord injury, neurological conditions, and neurodegenerative diseases, respiratory conditions, and gastro-intestinal conditions, including percutaneous endoscopic gastrostomy (PEG) feeding.
Key Responsibilities:
- Inbound and outbound calls to clients and workers to check welfare and the cover of shifts
- Keeping the Team Leader updated with business-critical information daily
- Ensuring compliance with company policies and procedures in all locations and achieving exacting standards of regulatory compliance
- Communicating effectively with staff at all levels, including those who use our services and members of the public
- Identifying and collaborating on innovative ideas and supporting a creative, flexible, and responsive workplace
- Developing and delivering best practice and ensuring workloads are met in a timely manner
- Representing the company and maintaining/developing positive relationships with internal and external stakeholders
Requirements:
- Experience as a Care Coordinator
- Previous experience of using booking and phone systems
- Ability to deal with conflict in a calm and supportive manner
- Strong work ethic and a dynamic, flexible approach to the role
- Excellent customer service skills
- Ability to work as a team, alone, and think outside the box
- Excellent communication and interpersonal skills
- Willingness to learn and develop
- Successful candidates will be required to undergo an Enhanced DBS
