We are currently seeking a highly skilled and compassionate Care Coordinator to join our team in the Thame area of Oxford. As a Care Coordinator, you will play a vital role in ensuring the seamless delivery of our care services, working closely with our clients, their families, and our support workers to provide a safe, reliable, and outcome-based care experience.

Job Summary

We offer a competitive salary of £25,500 - £30,000 per annum, depending on experience, plus a range of benefits, including a 25-day holiday entitlement, birthday off, and access to our Active Learning Hub, where you can develop your skills and knowledge through e-learning and face-to-face training opportunities.

About the Role

As a Care Coordinator, you will be responsible for:

  • Coordinating and scheduling care packages to ensure continuity of care for our clients
  • Providing a premium customer service, responding to client and worker queries, and resolving any issues promptly
  • Working closely with our support workers to ensure that our clients receive the highest level of care and support
  • Maintaining accurate records and reporting any concerns or issues to the Team Leader
  • Participating in the on-call rota, providing out-of-hours support and cover for our clients and support workers
  • Developing and maintaining positive relationships with our clients, their families, and our support workers

Requirements

To be successful in this role, you will need:

  • Experience as a Care Coordinator, preferably in a home-based care setting
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, their families, and support workers
  • A strong background in care coordination, with knowledge of relevant policies and procedures
  • The ability to work independently and as part of a team, with a flexible and dynamic approach to the role
  • Excellent time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities
