Job Title: Senior Care Coordinator

Job Summary:

We are seeking an experienced Senior Care Coordinator to join our team as a Complex Care Manager. The successful candidate will be responsible for delivering high standards of care to our clients in the Oxford catchment area.

Key Responsibilities:

* Conduct thorough assessments and implement personalized care plans
* Induct support workers and supervise team members
* Perform spot checks and risk management
* Attend meetings with ICBs and clients
* Provide regular reports and updates

Requirements:

* Hold an NVQ Level 3 (or equivalent) in Health and Social Care or a related topic
* Proven track record in managing and coordinating care for clients in a community setting
* Experience in developing and implementing care plans
* Supportive management style that can bring out the best in others
* Advanced communication skills verbally and written
* Full UK driving licence for client visits
