Complex Care Manager

Leaders in Care is seeking an experienced Complex Care Manager to join our team in Oxford. As a Complex Care Manager, you will be responsible for delivering high standards of care to our clients in a community setting.

Key Responsibilities:
  • Conduct thorough assessments and implement personalized care plans
  • Induct and supervise support workers
  • Perform spot checks and risk management
  • Attend meetings with ICBs and clients
  • Provide regular reports and updates
Requirements:
  • NVQ Level 3 (or equivalent) in Health and Social Care or a related topic
  • Proven track record in managing and coordinating care in a community setting
  • Experience in developing and implementing care plans
  • Supportive management style
  • Advanced communication skills
  • Full UK driving licence
