**Role Overview**
We are seeking a highly skilled Complex Care Manager to join our Bespoke and Team Brain Injury Support teams, part of the Acuity Care Group. In this field-based role, you will be responsible for managing support packages with varying requirements, requiring a high level of autonomy and decision-making.
**Key Responsibilities**
- Manage support packages with complex needs, ensuring seamless delivery and exceptional outcomes.
- Develop and maintain strong relationships with clients, their families, and internal stakeholders.
- Collaborate with multidisciplinary teams to provide holistic care and support.
- Conduct regular reviews and assessments to ensure support packages remain effective and relevant.
**Requirements**
- Proven experience in care management, with a focus on complex support packages.
- Strong knowledge of the care sector, including regulations and best practices.
- Excellent communication and interpersonal skills, with the ability to build strong relationships.
- Ability to work autonomously, with a high level of initiative and decision-making.
