Job Overview: Adult Care Worker (Reablement)

Care Outlook Ltd is seeking an experienced Adult Care Worker (Reablement) to join our team. As an Adult Care Worker (Reablement), you will be responsible for supporting vulnerable adults in the Oxfordshire area, mainly Cherwell, to regain independence following a hospital stay.

Responsibilities:
  • Complete assessments with new clients following discharge from hospital
  • Support clients to regain independence with tasks such as personal care, toileting, meal preparation, and medication
  • Record and report changes clearly and concisely
  • Build strong working relationships with clients, colleagues, and the Home First team
  • Be adaptive and responsive to changes in needs

What We Offer:
  • Estimated annual salary: £25,500 - £31,000 depending on experience
